With the reelection of Barack Obama, and Democrats retaining the majority in the Senate, a few things are clear:
Obamacare will not be repealed. It will continue to go into effect on schedule. Once fully implemented, I believe the major provisions will never be repealed--the law will only be fiddled with on the edges.
The Supreme Court is no longer in danger of being taken over fully by the conservatives. It may even tilt the other way by the end of these next four years.
Republicans will have to make concessions to avoid the fiscal cliff. They just don't have the luxury of saying "we're not giving an inch" this time around, because the do-nothing approach causes Bush's tax cuts to expire automatically. Do-nothing, in other words, equals a big tax increase. Republicans can either let that happen, or do a deal with Democrats.
What else? I don't know.. it remains to be seen how ongoing divided government is going to play out. Did Republicans learn any lessons from their losses, or will they press on with more of the same? Time will tell.
