The first one's confusing. There's some studies out that suggest obesity may not cost as much as previously thought. It depends I guess on what you mean by biggest cost to healthcare. If you mean just how much is spent each year, then I think it still is just because so many people are obese. If you mean how much an obese person costs over their lifetime, then no. Turns out dying before one gets into the really expensive old people care makes obese people good for insurance plans.

As for the second point, I think so. It's that or high blood pressure, although now those may be related as well.
